Referral Program

We appreciate the trust and confidence you have shown us! To show our appreciation, if you refer a friend or family member to our hospital (that has never been seen by us before) we will give them a free exam. In exchange, you will receive a $20 credit to your account with us for each person you refer.

All you have to do is follow these simple steps:

  1. Download and print our Referral Certificate. *You will need Adobe Reader to download/print this certificate. If you do not have Adobe Reader, please download it for free from www.adobe.com
  2. Write your client number on the certificate. Call us at (714)899-1100 for your client number if you don't already know it.
  3. Give the certificate, with your client number on it, to a friend or family member. It's that easy!

Remember, the referral certificates can ONLY be used by new clients and must be presented at time of their first visit.
